Surgical First Assisting Certificate (SFA Online Program) is a Certificate program (Hybrid) offered by Meridian Institute of Surgical Assisting in Nashville, TN. The program takes 12 months (52 weeks) to complete, with a total tuition of $8,045.
Program Overview
Meridian’s SFA Online Program is a 28 semester credit, 648.75 clock hour certificate that prepares working surgical technologists and other operating room professionals to become surgical first assistants. It is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) on the recommendation of the Subcommittee on Accreditation for Surgical Assisting of the Accreditation Review Council on Education in Surgical Technology and Surgical Assisting (ARC/STSA), and Meridian as a whole is institutionally accredited by ABHES.
The program runs in three phases over 12 months. Phase I is twelve online courses in the Canvas learning management system, covering surgical anatomy and physiology, microbiology, pharmacology, anesthesia, bioscience, the first assistant’s role and legal responsibilities, fundamentals, suturing, surgical procedures, complications and the business side of assisting. Phase II is the 60-hour Applied Science Lab, either a five-day on-campus lab in Nashville or a simulated lab by distance education. Phase III is a clinical externship of 140 surgical cases at your own facility under an approved preceptor.
The curriculum follows the Association of Surgical Technologists’ Core Curriculum for Surgical Assisting, 4th edition, and Meridian describes its approach as “why-to-how”: students learn why a task is performed before learning how to do it. The program prepares graduates to sit for the Certified Surgical First Assistant (CSFA) and Certified Surgical Assistant (CSA) national examinations, and CAAHEP standards require every student to schedule the CSFA exam before graduation.
What sets this Program apart
- Live-tissue surgical lab. The on-campus Applied Science Lab runs in six operating room stations, and procedures are performed on anesthetized pigs so students manage real tissue and real bleeding rather than rubber models.
- Learn while you keep working. Coursework is online with weekly deadlines, and the externship is completed at your own healthcare facility, where every student is expected to be employed.
- A fully distance option. Students who choose the simulated skills lab can complete the whole program without traveling to Nashville.
- Monthly starts and CAAHEP accreditation. Twelve cohorts start each year, and the certificate has held CAAHEP accreditation since 2005.

Career Outcomes
Graduates work as surgical first assistants, directly assisting the surgeon with exposure, hemostasis, suturing and wound closure across surgical specialties. The program prepares students for the Certified Surgical First Assistant (CSFA) examination offered by the NBSTSA and the Certified Surgical Assistant (CSA) examination, and many students are already employed in surgery while they train. Meridian does not guarantee employment, though it shares job openings it hears of from facilities nationwide.
Meridian’s Graduation and Placement Report for the reporting year July 1, 2024 to June 30, 2025 shows a graduation rate of 91 percent, a withdrawal rate of 9 percent and a placement rate of 80 percent for the Surgical First Assisting program. It reports an outcomes assessment exam pass rate of 87 percent for July 1, 2022 to June 30, 2023.
What to expect as a Meridian Institute of Surgical Assisting student
Meridian Institute of Surgical Assisting began operations in December 1999 and is based at 1507 County Hospital Road in Nashville, Tennessee. Its roughly 12,000-square-foot facility holds administrative offices, classrooms and a laboratory with six fully furnished operating room stations, equipped to run real-time surgical procedures comparable to those in hospital operating rooms. Meridian describes itself as the nation’s largest surgical assisting education program, and every program is taught in English.
Coursework runs in the Canvas learning management system, so you study from your own computer; the catalog lists the operating systems, browsers and internet speed Canvas needs, and Respondus LockDown Browser is used for testing. Direct student support is available Monday to Friday, 8:00 a.m. to 4:00 p.m. Central, email and online issues are monitored in the evenings and at weekends, and Meridian aims to answer every student inquiry within 24 hours. Instructors offer career advisement by appointment on employment opportunities, reimbursement, career pathways and professional development.
Meridian is authorized to enroll students in every state except California, which is not a member of NC-SARA, so California residents cannot enroll. Surgical assistants are not recognized in New York, New Jersey or Washington, which bars graduates from practicing there, and Nebraska, Virginia, the District of Columbia and Kentucky currently require professional licensure to work as a surgical assistant. Students must tell Meridian if they move to another state.
